Design a patient platform that connects appointments, preparation, documents, results, care plans, and family support across a fragmented healthcare journey.

The product needed to remain reassuring under stress, accessible across languages and abilities, and practical for clinics with different levels of digital maturity.

We replaced portal complexity with a time-based care journey. The home screen explains what is next, what to prepare, and who can help. Clinical information is progressively disclosed in clear, human language.
